Transportation services are
provided to active Medicaid recipients to the necessary providers
appropriate to client needs and only if other means of transportation
are unavailable. This also includes use of city bus being unavailable.

Requests for transportation
must be made at least two days prior to the appointment. Requests must
be made through Social Services

Transportation will only be
provided to the client who has the medical appointment except for
those needing assistance because of physical or mental disabilities.
Children must be accompanied by one responsible adult.

Transportation providers
will not carry children to the client's baby sitter nor will they baby sit
for client's children during the client's appointment. Clients must make
arrangements for children prior to the time the provider arrives to pick
them up.

When requesting return
transportation, the client must be at a designated area when finished
with their appointment. The drivers are not responsible for finding the
client and will leave the client if necessary. Clients will then be
responsible fir their own return transportation. If client leaves with
someone else without contacting DSS or CTS (291-1979) advising that a
ride home is not needed, the client will be assessed as a No Show and
Rile #14 will be applied.

Clients are requested to
make out-of-county medical appointments as early as possible and not
later than 1:00pm. Transportation will not be provided for Friday
afternoon out-of-county medical appointments. This will be enforced

Client should be ready to go
at least one hour prior to appointment time for in-town transportation
and two hours for out-of-county transportation.

The driver will wait for
client a ,maximum of 5 minutes when picking up.

The driver will perform only
transportation authorized by social worker and volunteer services
coordinator. Client may not be taken to any other appointments or
places, and no one else, other than those stated in Item #3 may ride.
You must state if your children have to go with you (see #3 above).

Client may not smoke, eat,
or drink in vehicle unless approved by driver.

Client should inform social
worker in advance, as much as possible, when appointment time changes or
when client decides not to go. If clients do not show for appointments,
Rule #14 will be applied as a No Show.

Clients in wheelchairs must
be secured to the wheelchair before entering the van.

Drivers provide
transportation services only. They are not responsible for bringing the
client to the vehicle. Proper arrangements must be made in advance for
clients needing assistance.
