Diabetes Self Management Education

Diabetes Self Management Education 

  • Available for newly diagnosed diabetics and/or diabetics who are having difficulty managing their diabetes.
  • Program requires a referral from the patient’s medical provider.
  • Program utilizes the American Association of Diabetes Educators Curriculum  

What is Included 

  • An initial patient assessment and education plan (1hour)
  • 8-hours of group education
  • A 3-month group follow-up session (1hour)
  • Annual individual session (1 hour)
  • Monitoring HgbA1C at the time of referral and at the 3-month follow-up
  • Written communication with the patient’s provider regarding individual goals, plans for ongoing self management support and outcomes

How to schedule an appointment:
To begin classes, you must first get a signed referral from your primary Physician, Physician's Assistant or Nurse Practitioner, which contains health information, laboratory results and insurance information.  The referral form is obtainable on our website so that you may take it to your doctor and ask them to fax it to our agency at (252) 293-8301.
